Taking the first step

The first step in getting support for a patient or loved one is to make a referral. Our referral and admissions coordinators can answer any question you have and support you in finding the right services for the person you support. We’ll handle everything from intake paperwork and insurance to scheduling medical screenings and transfers. Just give us a call to get started.

What happens next?

We’ll start gathering important information about the person we’ll be serving and schedule a clinical screening. This usually happens within 24 hours of receiving the referral. After the evaluation, we’ll arrange for the treatment team and family to talk about the proposed plan of care.

We’ll then work with the family to get information about funding qualifications and arrange for tours of the program. We’ll also submit the proposed plan of care to the funding source for approval. Once approved, we’ll help with arrangements to transfer and admit the person to one of our programs.
